Clarifications and deviations summary: cover sheet template

The one-page cover sheet that goes on top of a technical deviation list: the offer, the documents reviewed, the counts, and the items that need the customer's answer.

By Ademola Afolabi, Founder, New Odyssey Ltd · Last reviewed:

The short answer

This free cover sheet template goes on top of your technical deviation list. It names the offer and the documents reviewed, states that no other deviations are taken, counts deviations and clarifications, and lists the items that need the customer's written answer. XLSX or CSV, no sign-up.

Key Takeaways

  • The counts on the cover sheet update from the Items sheet.
  • List every document and revision reviewed, including addenda.
  • Keep "No other deviations are taken" only if it is true.
  • Send the same cover sheet and list in the priced and unpriced copies.

What it is for and who uses it

Evaluators read the cover sheet first. It tells them which documents and revisions you priced against, how many deviations and clarifications to expect, and which answers you need before the price and delivery hold. The proposal or bid engineer who wrote the deviation list fills it in, and the engineering manager or lead engineer signs it off.

The rows summarise the deviation list example. The file also has Response needed by and Status date columns. Every example value is invented: specification PS-VAL-001, the requisition, the datasheets and every certificate and report number do not exist. Standards such as API 6D, API 607, ISO 15848-1 and EN 10204 are named but not quoted, so check each clause against your own licensed copy.

How to use the template

  1. Send this sheet on top of the technical deviation list, in both the priced and unpriced copies of the offer.
  2. Fill in the header block. Give the deviation list number and revision, and state the order of precedence the requisition sets, or say that you raised it as a clarification.
  3. Edit the statement so it says exactly what your offer covers. Keep the sentence "No other deviations are taken" only if it is true.
  4. List every document and revision you reviewed, including addenda, on the cover sheet.
  5. On the Items sheet, add one line per deviation list item that is a Clarification, Deviation or Not offered, with the date you need an answer by.
  6. The summary counts on the cover sheet update from the Items sheet.
  7. When the customer replies, record the response and status date here and in the deviation list, and reissue both with a new revision.

Frequently asked questions

What goes on a deviation list cover sheet?

The customer, project, RFQ and offer numbers, the documents and revisions reviewed, a statement that no other deviations are taken, counts of deviations and clarifications, and the items that need the customer's written answer.

Do I need a cover sheet if I already send a deviation list?

It is not always required, but it helps the evaluator. It confirms which document revisions you priced against and puts the open questions on one page.

Should the cover sheet go in the priced or the unpriced offer?

Both. Some buyers ignore deviations that appear only in the priced copy, so the cover sheet and the deviation list should be identical in both.
